R.M.A. FORM

This form is for Warranty & Damage Claims and Returns.  You must fill out one of these forms to get an R.M.A. number before you return your merchandise. 

If you return any merchandise without approval it will be refused and returned to you.

 

You will be notified by e-mail or fax within 48 hours.  If you do not give us an email address or fax number, you cannot be notified about your return.

IF YOU DO NOT GIVE US AN ACTIVE EMAIL ADDRESS YOU WILL NOT RECEIVE AN R.M.A. 

IF YOU DO NOT RECEIVE AN ANSWER WITHIN 48 HOURS PLEASE EMAIL US

R.M.A. is Void if items are not received within 10 Days of our emailed Issued  R.M.A. number. If you do not include the R.M.A. number on the box, it will be refused and returned to you.
